Full Mouth Reconstruction
Treatment Variations
There are many ways of treating patients depending on budget, preferences, medical conditions, doctor’s opinion, bone quality, infections, and treatment lengths
Category 1
Excessive wear with Loss of Vertical Dimension
A typical patient in this category has few posterior teeth and unstable posterior occlusion. There is excessive wear of anterior teeth. Closest speaking space of 3mm and interocclusal distance of 6mm. There is some loss of facial contour that results in drooping of the corners of the mouth.
Patients with dentinogenesis imperfect with excessive occlusal attrition, around 35 years of age, and appearing prognathic in centric occlusion also belong to this category. The closest speaking space of 5mm and interocclusal distance of 9mm indicates there is a loss of occlusal vertical dimensions with concomitant occlusal wear.

Category 2
Excessive Wear without loss of vertical dimension of occlusion but with space available
Patient has adequate posterior support and history of gradual wear. Closest speaking space of 1 mm and interocclusal distance of 2-3 mm. Continous eruption has maintained occlusal vertical dimension leaving insufficient interocclusal space for restorative material.
Manipulation of mandible into centric relation will often reveal significant anterior slide from centric relation to maximum intercuspation.

Category 3
Excessive wear without loss of vertical dimension of occlusion but with limited space available
Posterior teeth exhibit minimal wear but anterior teeth show excessive gradual wear over a period of 20-25 years. Centric relation and centric occlusion are coincidental with closest speaking space 1mm and interocclusal distance 2-3 mm. It is most difficult to treat because vertical space must be obtained for restorative material.

If you are not in either one of these 3 categories, you are currently wearing dentures.
Please look at our Treatment Alternatives
Infection, Missing teeth, treatment times, budget, and availability to travel all contribute to your treatment plan. Treatments plans for full mouth restorations can only be accurately done in the office after being seen by Dr. Irma Gavaldon D.D.S M.S A.E.G.D. Please read our next section which will cover how to diagnose and do a treatment plan for these cases.
Once Dr. Irma Gavaldon obtains all information relevant to the dental treatment, she will develop a comprehensive treatment plan to correct all the problems in your mouth and complete your full mouth reconstruction. If you have any questions about the procedure or you don’t understand the procedure being described to you, ask us for a detailed written description of the proposed treatment plan so you can review it. This can be helpful if you want to get a second opinion. Be sure you understand the risks and benefits of the recommended procedures and treatments.

Dental Implants Treatment Process
Surgical phase step by step

Step 1
Consultation with our dental specialists
Oral and systemic health assessment • Facial esthetic evaluation • X-Rays, Panoramic, 3D CBCT Scan (full in-house equipment) • Wax-up (important for an aesthetic outcome for your implant treatment) • Informed consent • Treatment options (restoration materials, implants) • Treatment alternatives

Step 2
Preoperative planning for the best outcome
Our specialists always plan these cases with the most advanced implant planning software • Instruction and motivation on maintaining a proper oral hygiene (antibacterial mouthwash, such as Chlorhexidine 0.12% is recommended) • Record of maxillomandibular relations and occlusion • Impression used for fabricating the provisional prosthesis • Surgical guide 3-D printed in-office • Medication (antibiotics and sometimes sedatives)

Step 3
Anesthesia Options – No pain dentistry
Local anesthesia is always required (at no cost) • We recommend conscious sedation for these procedures (certified anesthesiologist $500 USD) • Pill Sedation (at no cost) • Choose what option is best for you

Step 4
Preprosthetic surgery
Teeth extraction, Aveloplasty (bone leveling removal of infected tissue) • Alveolectomy (when ridge crest is displayed during smiling) • Bone grafting procedures

Step 5
Implant placement
Implant number, position and design (diameter, length) • Usually, first the mesial implants are placed and last the distal implants • All on 4, All on 5, All on 6, All on 8 • Exposure of implant site – flap or flapless technique

Step 6
Additional surgical procedures
Bone grafting (socket grafting, with autograft and synthetic alternatives, with or without membrane) • Suture

Step 7
Interim prosthesis
Abutments selection (angulation) • Impression and record of maxillomandibular relationship • Design – dental arch length (number of teeth, usually 12 per arch) • Materials – acrylic or PMMA

Step 8
Postoperative instructions
Radiological exam • Instructions for oral hygiene, diet (soft diet, for at least 10 weeks) and medication (antibiotics, analgesic drugs) • Informing about the need to make an appointment if bleeding, pain, implant mobility, detachment or damage of the prosthesis occur • Establishing the next appointment

Step 9
Definitive prosthesis
Design – dental arch length (number of teeth, usage of cantilever extremities) occlusion scheme, fixation type as occlusal screw-retained • Materials

Step 10
Maintenance
Regular check-ups • Addressing complications

Dental Implants Pre-Op Instructions
before coming to Cancun
history Before surgery (three weeks)
Smoking and dental implants don’t go together. Why did you lose your teeth/tooth in the first place? We know smoking can be difficult to quit but we insist that patients stop smoking at least three weeks before their dental implants. On unusual occasions, we would ask you to stop smoking sooner, because it can interfere with your recovery, by reducing blood flow, which may also prevent the wound from nicely healing. You should not smoke until our dental surgeon authorizes you.
history Before surgery (three weeks)
We know you will be on vacation and the all-inclusive bar might be tempting. You must stay away from any alcoholic beverage for a minimum of five days before your procedure. Alcohol causes increased bleeding, and the chances of infection, as well as depress immune functions and might interfere with wound recovery.
history 3 weeks before surgery
Over-the-counter and prescribed
Please check with your physician about suspending any medication prior to arrival.
Our dental surgeon or patient coordinator will inform you to temporarily stop some medications within the weeks ahead.
history Before surgery (three weeks)
It is essential that you maintain a healthy food plan. You will be on a soft mechanical diet or modified and adapted to your surgery, so you must follow these instructions. Get protein supplements like Ensure or Boost, as well as soft foods like well-cooked pasta, yogurt, eggs, broths, and soup. We will provide you with a list of dietary suggestions in your post-op packet as well.
history Before surgery (two weeks)
- You must consult with your physician before discontinuing any medication.
- All anti-inflammatory drugs for arthritis (remember to consult your physician).
- Any kind of anti-coagulants.
- Vitamin E took internally.
- If you have any doubt about these medications, please contact us
history Before surgery (two weeks)
During this period, we recommend you to take:
- A multivitamin tablet – daily (most multivitamins contain some form of vitamin E, however, this is suitable to take before surgery. We will ask to avoid taking any extra vitamin E).
- Vitamin A – twice a day: 8,000 to 10,000 units (this has well-documented beneficial effects on surgical healing and also will help your immune system).
- Vitamin C – two times a day: 1,000 mg (after your surgical procedure your need for vitamin C will increase, and it is important for collagen synthesis, which helps to wound recovery).
- Pineapple enzyme – twice a day (you find it on health food stores and it is a great help to relieve the swelling related to a surgical dental procedure. This anti-inflammatory promotes healing).
history Before surgery: One week, until two weeks after surgery
You must eat:
- On each meal: a small amount of protein.
- Daily: three green vegetables and two fruits (according to diet instructions).
- Frequently: healthy meals.
history Before surgery: One week
- You may begin taking a topical anti-inflammatory every eight hours as directed and continue for 10 days.
- Every 12 hours between meals, take pineapple enzyme as directed (it is an anti-inflammatory and helps to heal). Fresh pineapple is a good alternative.
Post Dental Implant Surgery
Instructions
Standards of good oral hygiene are critical for the success and functionality of the implants and restoration. The best way to prevent infection and ensure healing is to keep your mouth clean. Clean your mouth thoroughly after each meal beginning the day after surgery. Use a soft-bristle toothbrush and toothpaste after meals and at bedtime. The use of a dental Waterpik is highly recommended to irrigate underneath the denture to flush out debris.
After a dental implant surgery is normally pink or blood-tinged saliva for two or three days and does not indicate any problem, even minimal bleeding is expected. In this case, the best way to stop it is by placing a damp piece of gauze and softly biting for 30 minutes while rest keeping your head elevated. In case the bleeding continues, repeat the gauze step for an additional 30 minutes. The suture material may last from two to three weeks, and you should not pull or cut the sutures by yourself. The surgeon will ask you to return to the clinic for a two-week follow-up appointment, so any sutures remaining at that time should be removed.
After any surgery, swelling is a normal reaction of the body and the first step to healing. In the beginning, swelling will not become visible until 24 hours after your dental implant surgery, and couldn’t reach its peak for two or three days. After this period, the swelling decreases but may persist from seven to 10 days. You can minimize swelling by applying immediate ice packs to the outside of the face for 20 minutes on, and then 20 minutes off while awake for the first 24 hours.
First of all, remember that all medications must be prescribed and should not exceed the recommended dosage. Because most oral surgery is followed by some amount of discomfort, the surgeon may suggest taking any anti-inflammatory, non-steroidal medication (only if YOU ARE NOT ALLERGIC to it), before the local anesthetic wears off. In case of intense pain, you should require calm pain medication under prescription, which will cause you to feel drowsy, so you shall not operate any mechanical machinery or drive while taking the prescription. About the post-operative antibiotics, they must be taken as directed and the entire course of the prescription. For the first weeks after your dental implant surgery, you could also be prescribed an antibacterial mouth rinse which must be used two times a day.
We suggest starting with liquids, after a general anesthetic or conscious IV sedation, and must avoid hot meals or hot liquids while numb. The limited food intake that is required for the first days could be compensated by increasing the fluid intake, with high protein shakes to replace meal supplements. For the first six weeks you must follow a soft chew diet, so if you cannot cut easily any food, please avoid eating it.
history 48 hours after your dental implant surgery
For the first 48 hours after your dental implant surgery, you must rest and relax without any physical activity. After these 48 hours, you should resume activity as tolerated.
Eat:
- Gelatin
- Yogurt
- Pudding
- Cream of wheat / Oatmeal
- Mashed potatoes
- Soups
- Ice cream
- Cottage cheese
- Applesauce
- Scrambled eggs
- Fish
- Pasta (well-cooked)
- Pancakes
Drink:
- Water
- Popsicles
- Protein shakes / Smoothies
- Juice
- Milkshakes
When numbness is gone you can increase diet as tolerated by adding soft food
- Try to keep your lips moist with special ointments (such as Vaseline) to avoid the corners of your mouth drying and cracking.
- Sore throats and pain when swallowing are not uncommon. The muscles get swollen and the normal act of swallowing can become painful. This will subside in 2 to 3 days.
- Stiffness of the jaw muscles may cause difficulty in opening your mouth for a few days. Do not force your mouth open. Massage the muscles and apply moist heat.
- With upper jaw implant treatment, you may develop bleeding from the nose. This is usually a temporary finding and will not persist past 2 to 3 days. Avoid blowing your nose for several days.
- An appointment with your general dentist may be needed within a few days of surgery to make any adjustments to your temporary denture.
- Your case is individual as no two mouths are alike. Discuss all questions or concerns with your oral surgeon or implant coordinator.

Dental Implants FAQs
Because by choosing your dental implants in Mexico, you will enjoy savings up to 70% off than in the U.S.A. and Canada, state-of-the-art facilities, accurate, faster dental treatments by certified specialists and even travel assistance.
The actual implant process is done in one day, but we like to have a few extra days so that we can check on your progress before you go home (five to seven days). The more time you can give us the better (for complicated cases including extractions, full mouth cases, or upper and/or lower dentures needed seven days minimum). We like to factor in a small margin of error in case you need your denture adjusted or to fix a loose stitch, whatever the case may be.
We are not the type of clinic to get you in and out of the door as soon as possible, the doctors want to make sure that you healing correctly before you return home.
This is called “immediate load” and it is possible for people who are candidates. You must have very good bone quality but even then, we cannot tell if you are eligible for an immediate load until the actual surgery.
If it is possible and this is what you want then we will do so, but if the risk of failure is too high we will not risk it. We want the best outcome for your case.
Yes, the implant needs time to integrate with the bone before we can rehabilitate it. This usually takes about 4-6 months.
If you cannot come back what we can do is send you home with the corresponding abutment so you can have it placed at home when you are ready.
Our clinic is a specialty clinic, so you will be dealing with our Maxillofacial surgeon and implant specialist Dr. Javier Paz.
Dr. Gavaldon is our restorative specialist and the one who will be restoring your implants.
Dr. Javier Paz has many years of experience in oral surgeries and has had great results in the time he has been with us.
We will send Dr. Paz’s credentials to your Email, so you can verify them. He places implants with us on a weekly basis with great results, if you like we can send you a few references.
We guarantee our work for 5 years. If there is a problem that arises due to a fault on our end we will give you free accommodations on your return and replace the implant at no cost.
We are very responsible in each case we handle. This is why we work only with trained specialists, to make sure that your work is done correctly and to avoid any hassles in the future.
We work with Noble Biocare, Straumann, Implant Direct, and MIS. All of these implants are full titanium and are from known and well-respected brands. These are the same implants that the U.S and Canadian dentists use, and are not some cheap material.
Yes, we can place your dental implants after extracting your teeth, but it would depend on your remaining bone structure after extraction. In a lot of cases, we will place the bone graft and implant after extracting the tooth.
We would have to see an X-ray to determine this for certain. In these cases, some bone grafting and the membrane are usually required.
We use good quality material on our dental implants, so unlike some other clinics in Cancun, we cannot slash our prices.
We also provide all of your transportation once you arrive in Cancun so that you do not have to spend extra cash on taxis and buses.
Also, keep in mind that our prices are all-inclusive, so we do not charge you for periapical X-Rays, consultation, dental-related tests, specialist fees, taxes among others.
Treatment Alternatives
All On 4
If you have teeth that are mobile, missing in different locations, or have an infection or serious decay, you may consider a long-term approach to your dental problems by extracting these poorly functioning teeth and performing the All on 4 procedure. With a Fixed denture stabilized with a titanium bar and secured to 4 Implants, you will have the stability and esthetics that you were lacking.
Orthognathic Surgery
Perhaps your dental issues stem from a joint problem called TMJ and you do not want to crown your teeth in order to restore the bite. Our Maxillofacial Surgeon is qualified to perform Orthognathic Surgery to help correct these TMJ issues. Orthognathic Surgery is usually done after or before an Orthodontic treatment. Contact a Dental Coordinator for questions regarding your case.
Zygomatic Dental Implants
Zygomatic dental implants are implants that are much longer than conventional implants (40 mm – 50 mm), and are placed in the dense Zygoma bone or otherwise known as the cheekbone. The advantage of using Zygomatic Implants is that it eliminates the need for procedures like ridge augmentation (Bone Grafting) and/or Sinus Lifting, which would require more surgery and longer healing periods before the placement of the final restoration.
